With the advent of an aging society, elderly patients face challenges from multiple comorbidities and complex care needs. To enhance medical quality, our hospital developed the "Smart Integrated Care Model for the Elderly." This comprehensive smart healthcare application integrates structured electronic medical records, multidisciplinary case management, functional assessment tracking, risk stratification alerts, and home physiological data synchronization. To date, this system has served over 15,000 cases. Users’ surveys indicate a satisfaction rate of more than 80% among healthcare teams with the integrated system, with the risk alert feature achieving 83% satisfaction, demonstrating high clinical utility and precision.
This system facilitates clinical decision-making through a "One-Stop Smart Care View." This interface allows healthcare providers to view laboratory results, questionnaire assessments, and physiological data side-by-side, utilizing visual trend charts to accurately track the health trajectories of older patients. This approach not only eliminates fragmented care but also enables the medical team to proactively identify potential risks and implement personalized health management.
For patients, this model provides seamless, continuous care, reducing redundant assessments and communication gaps while enhancing the sense of security during treatment. For healthcare providers, the built-in "Care Assistant" integrates automated tracking, cross-departmental reporting, and smart reminders, significantly reducing administrative burdens and simplifying reporting workflows, allowing clinicians to refocus on core professional care. From a societal perspective, this system optimizes integrated care through technology, reduces healthcare expenditures associated with adverse, events and establishes a high-efficiency paradigm for smart medicine. By narrowing the gap in specialized care through intelligent management, this model realizes person-centered continuous medical care services, laying a solid foundation for the quality of care for older adults.
